Job summary

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Knowledge of patient-/family-centered practice and collaboration with patients, families and care partners.
• Cultural awareness and respect for diverse backgrounds.
• Effective communication with patients and families; appropriate delegation.
• Knowledge of patient assessment and care planning.
• Knowledge of discharge planning and post-discharge needs.
• Health information documentation and charting.
• BLS certification and PA RN licensure required.

Responsibilities

• Interacts with patients, families, co-workers, and staff to deliver patient-centered care.
• Implements nursing process and evidence-based practices.
• Educates patients and families on learning priorities and care plans.
• Minimizes risk by following established nursing protocols and performance.
• Uses information systems to document care and communicate effectively.
• Monitors care outcomes and supports quality improvement.
• Collaborates with inter-professional teams for safe, coordinated care.
• Participates in hospital and practice committees on safety and quality.
• Mentors appropriate staff and acts as preceptor when needed.
