Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered practice in care planning.
• Cultural awareness and ability to work with diverse populations.
• Effective communication with patients, families and staff using appropriate behaviors.
• Knowledge of discharge planning and post-discharge needs.
• BLS certification required.
• RN license active in PA state.

Responsibilities

• Collaborate with patients and families to develop individualized care plans.
• Educate patients and families on learning priorities and care steps.
• Use IT to communicate, monitor outcomes, and support safe care decisions.
• Work within nursing and inter-professional teams to provide safe, compassionate care.
• Participate in safety/quality improvement committees and act as a preceptor for staff.
