Job summary

Advocate Health seeks a Nurse Navigator in Cardiology: Electrophysiology to coordinate patient care from diagnosis through follow-up. You will educate patients, coordinate tests and appointments, and ensure seamless access to resources.

The role emphasizes collaboration with electrophysiology, imaging, anesthesia, pharmacy, scheduling, and referring clinicians. BSN is required with NC RN licensure; MSN is preferred.

Qualifications

• BSN Degree required; RN license in the state of North Carolina.
• MSN preferred.
• Excellent communication and patient education skills.
• Strong coordination of care across multiple teams and departments.

Responsibilities

• Coordinate patient care from diagnosis through follow-up to improve efficiency and quality.
• Serve as patient advocate and liaison to ensure access to resources and services.
• Educate patients and families about diagnosis, work-up, treatments, and follow-up.
• Coordinate referrals, testing, scheduling, and pre/post-procedure instructions.
• Identify barriers to care and support continuity of care across settings.
