The Pennant Group, Inc. is seeking an Electronic Referral Specialist to serve as the initial contact for electronically received referrals across our home health and hospice network in Nashville.

You will monitor portals, review referrals for service type and payer alignment, and route them to the appropriate agency location to ensure timely processing. Competenze

• Minimum of 1 year of healthcare office, patient access, scheduling, intake, or referral management experience.
• Experience working with electronic referral platforms (CarePort, Forcura/Mosai, hospital EHR feeds).
• Familiar with EMR systems; Homecare Homebase (HCHB) knowledge is a plus.
• Basic knowledge of Medicare, Medicaid, and commercial insurance terminology.
• Understanding of home health and hospice service types and documentation expectations.
