Job summary

Bluegrass Care Navigators is seeking a PACE Social Worker to provide and coordinate psychosocial services for participants and their families across diverse settings, including the PACE Center, home, hospital, and ALF. The role emphasizes holistic care and strong collaboration with an interdisciplinary team.

You will deliver community-based case management and work with a network of local resources to support participants’ well-being and independence.

Qualifications

• MSW degree; licensed in Kentucky (CSW or LCSW).
• Minimum two years in social services in a case management role.
• At least one year of experience with frail and elderly populations.
• Proven ability to network with community resources.
• Strong communication, organizational, and interpersonal skills.
• Ability to manage multiple priorities with discretion.

Responsibilities

• Provide and coordinate psychosocial services for participants and families in the PACE Center, home, hospital, or ALF.
• Offer community-based case management and support services for program participants.
• Collaborate with interdisciplinary team to determine best care plans and resources.
