Job summary

Alder is seeking a Life Care Manager to support aging families in the Greater Boston area. This remote full-time position involves professional assessments, care planning, and advocacy, coordinating medical care, and guiding families through aging-related decisions.

Ideal candidates have an advanced degree with licensure in a relevant field, 3–5 years of experience with older adults, and strong interpersonal skills.

Qualifications

• Advanced degree preferred and licensure required in social work, nursing, geriatrics, mental health, or a related field.
• 3–5 years’ experience working with older adults and families.
• Excellent clinical skills; knowledge of family dynamics and family systems.
• Highly organized and able to manage fee-for-service caseload effectively.
• Outstanding interpersonal and communication skills.

Responsibilities

• Conduct professional, objective assessments of older adult clients and their families, including psychosocial history and in-person meetings.
• Develop professional relationships with clients, families, and care providers across care settings.
• Create and communicate client-centered, objective care recommendations and resources to clients and families.
• Assist in the implementation and oversight of care recommendations (home care, assisted living, nursing home, etc.).
• Provide client-centered problem-solving, ongoing support, and advocacy.
• Arrange for and accompany clients to medical appointments and arrange transportation as needed.
• Provide support and education to family coping with loss, dementia, illness, or change.
• Document billing and interventions daily.
• Attend weekly supervision with the Director of Clinical Services and monthly staff meetings.
• Pursue licensure and certification per ALCA and MA requirements.
