You will conduct home visits, collaborate with an interdisciplinary team, and help educate the community on elder abuse mandates. Qualifications

• Bachelor's Degree in Human Services or related field with 2 years' related experience.
• Experience with high risk/crisis situations in Case Management or Behavioral Health.
• Flexibility for rotating schedule, on-call after hours and weekends.
• Effective documentation, timely and accurate.
• Excellent communication with team, consumers and partners.
• Valid driver's license and reliable transportation.
• Knowledge of community resources preferred.
• Ability to work in interdisciplinary team.
• Proficient in Windows software (Word, Excel, PPT, Outlook).
• Confidentiality and HIPAA compliance.
• Bilingual a plus.

Responsibilities

• Receive and review intake reports on elder abuse; respond with supervisor support.
• Complete thorough investigations of abuse allegations and risks.
• Evaluate functional capacity and resources; arrange competency evaluations if needed.
• Pursue legal action such as protective orders and guardianship through courts.
• Educate community and providers on elder abuse mandates.
• Conduct home visits as necessary; participate in on-call coverage.
