Job summary

Leech Lake Band Of Ojibwe seeks dedicated staff to support elders in staying in their homes and communities. You will develop care plans with elders and involve family members to ensure immediate needs are met.

Responsibilities include home visits, accurate documentation, and helping elders access daily living resources. The role requires HIPAA training, cultural sensitivity, and the ability to travel as needed within the Reservation and surrounding counties.

Qualifications

• High School Diploma or GED required.
• AA degree in Gerontology or related health field preferred.
• Four years’ experience working with elders preferred.
• CHW certification or obtain within the first year.
• HIPAA compliant and data privacy training annually.

Responsibilities

• Perform home visits and coordinate elder care plans with elders and active family members.
• Accurate documentation of elder services/visits.
• Assist elders in finding daily living resources.
• Provide transportation to enrolled elders as needed.
• Access programs within the Reservation/Counties for elder needs.
• Coordinate social activities for community elders.
• Report elder needs/activities to the Director.
• Maintain confidentiality and professional ethics at all times.

Worksite: Elder Services Office and surrounding Reservation Communities

LLBO Status: Non-exempt

Safety Sensitive: Yes

License/Certification Requirements: Valid MN Driver’s License, current insurance or be insurable.

Summary: Support elders who wish to remain living in their homes/communities. Develop plans with the elder that adequately will meet their immediate needs making certain that family members are active participants.

Education Requirements and Experience:

High School Diploma or General Education Diploma (G.E.D) is required.

A.A. Degree in Gerontology or related Health field preferred.

Four (4) years’ experience working with elders preferred.

Have CHW Certification or obtain within the first year.

HIPPA compliant and data privacy training annually.

Additional Qualifications and Skills:

Understanding of the Ojibwe Culture and language in communicating with elders preferred, but not required.

Good communication and customer service skills.

Ability to identify and access the needs of the enrolled Elders of the reservation.

Possess the ability to work independently, with minimal supervision, in order to ascertain individual elder needs.

Work well within a team setting and manage time effectively.

Be sensitive to the elder needs and explore all resources available.

Must possess professional integrity.

Native American Preference.

Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe positions.

Duties/Responsibilities:

During home visits coordinate a care plan with elders, and active family member .

Perform home visits.

Accurate documentation is required for each Elder services/visit.

Advocates will assist with finding resources for the Elders in need of daily living services.

Transportation will be provided to enrolled elders as needed, when all other options have been exhausted.

Act as a resource, assist and / or access the programs, within the Reservation/Counties that pertain to elders needs.

Coordinate social interaction for community elders.

Provide the Director with regular reports on elder needs and activities.

Must maintain confidentiality at all times regarding clients/staff.

Assure all administrative matters are reported the Elder Services Director.

Work cooperatively as an Elder Advocate team member and promote teamwork within the Elder Care Team.

Maintain a professional attitude.

Follow GAPP policies governing staff conduct.

Attend and or conduct all mandatory meetings and trainings as required.

Maintain confidentiality and professional ethics at all times.

Travel in and out of the state on a possibly regular basis.

Understanding and sensitivity to Native American culture.

Physical Requirements:

Occasionally be able to lift up to 10 pounds.

Sedentary work with some standing while filing, copying, etc.

Must be able to decipher inclement weather.

Subject to internal working conditions.

Must be able to operate a motor vehicle safely in adverse weather conditions.
