Job summary

Leech Lake Band of Ojibwe seeks a Housing Stabilization Case Manager to guide individuals and families through person-centered housing planning within the LLBO Homeless Services Program.

The role requires an Associate degree in human services (or equivalent experience), HMIS certification, and strong skills in organization, communication, and confidentiality. Flexible 40-hour weeks and coordination with tribal resources are expected.

Qualifications

• Associates Degree in human services or related experience preferred.
• Experience in group/residential settings is preferred.
• Must obtain CPR/First Aid, NARCAN, AED within 3 months.
• Must obtain Mandated Reporter certification within 3 months.

Responsibilities

• Perform and complete Housing Focused Person-Centered Plan with residents.
• Manage and assist residents in permanent housing acquisition, updating plans annually.
• Connect residents with services such as substance abuse assessment or treatment.
• Educate residents on risks related to substance abuse or Behavioral Health.
• Assist with housing transition and stabilization activities.
• Transport for necessary housing transition and stabilization appointments.
• Data entry, reporting and billing for Housing Case Manager duties.
• Refer residents to community and supportive services.
• Ensure referrals for housing supportive services are completed.
• Be familiar with policies, maintain confidentiality.

Job description

License/Certification Requirements: Homeless Management Information System (HMIS) certified. CPR and First Aid for adult, child, and infant, NARCAN and AED certifies. Mandated Reporter certified. Valid MN Drivers License, current insurance or be insurable.

Summary: Will serve as a housing stabilization Case Manager and facilitator for individuals and families of the LLBO Homeless Services Program, performing all housing Person Centered Planning operations with residents.

Education Requirements and Experience:

Associates Degree from an accredited college in human services field preferred, or equivalent experience directly related to transitional and permanent housing Case Management.

Experience in group/residential setting preferred.

Must have or obtain CPR and First Aid for adult, child, and infant, NARCAN and AED certification within three months of employment.

Must have Mandated Reporter certification or obtain certification within three months of hire date.

Must have DHS Housing Consultant Training for Housing Stabilization or obtain training within three months of hire date.

Must be Homeless Management Information System (HMIS) certified within three months of employment.

Additional Qualifications and Skills:

One years’ experience working in the human services/social services field preferred.

Effective team player who is honest, non-judgmental.

Ability to handle multiple tasks, prioritize needs and follow through.

Must have strong organizational skills, the ability to work independently and work with the Site Manger to assure quality of services provided.

Ability in the area of communication skills including strong writing (documentation), speaking and listening skills.

Ability to work with people from varied cultural, economic and educational backgrounds through the various curriculum and services provided by the organization.

Knowledge of Tribal, County and State resources helpful.

Skill in the use of computers and commonly used office software.

Native American preference.

Must be able to work a flexible 40-hour week schedule.

Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe positions.

Duties/Responsibilities:

Perform and complete Housing Focused Person-Centered Plan with residents.

Manage and assist residents in permanent housing acquisition, updating plans as needed and annually.

Connect residents with services such as Substance Abuse assessment and or Treatment, or Behavioral Health assessment and or treatment services including Opiate Replacement (MOUD) if necessary.

Educate or facilitate resident’s education related to risks associated with substance abuse or Behavioral Health disorders including HIV, Infectious disease concerns and prevention of exposure to them.

Assist residents with housing transition and Housing stabilization activities including any activity which supports permanent housing acquisition.

Transport for necessary housing transition and stabilization appointments.

Responsible for data entry, reporting and billing for Housing Case Manager duties in a timely manner.

Refer residents to any community and supportive services available for residents.

Ensure necessary referrals for housing supportive services are completed.

Ensure flow of critical information including documentation and shift change to bring forward issues and occurrences that take place while on duty.

Plan and implement program activities for Homeless Shelter guests.

Support guests, monitor, and assess behavior and well-being. Assist with life building skills.

Effectively de-escalate, diffuse and monitor overall shelter environment for safety.

Collect and submit data on all guests for HMIS and LLBO.

Participate in appropriate staff training programs and participate in both internal and external meetings.

Prepares reports as requested by the Shelter Manager.

Attends other meetings, trainings, seminars and conferences as necessary.

Be familiar with and implement all policies and procedures approved by the RBC.

Must represent the Homeless Services Program in a professional manner at all times, on site and in the community.

Maintain confidentiality and professional ethics at all times.

Must be able to exhibit understanding and sensitivity to the Homeless Population.

Adhere to policies governing staff conduct.
