Qualifications

• Must be organized, self-starter and detail oriented.

• Demonstrate adaptability to changes in healthcare environments.
• Navigate and analyze data for quality improvement initiatives.
• Experience with population health programs is preferred.

Responsibilities

• Evaluate electronic workflows and population health data.
• Identify barriers to preventive care completion.
• Design and collaborate to implement standardized EHR workflows.
• Support quality improvement initiatives.
• Maintain and revise EHR templates.
• Optimize EHR clinical decision support tools.
• Improve EHR quality measure documentation workflows.
• Support HEDIS, UDS, and payer initiatives.
• Develop training programs on population health/quality EHR workflows.
• Provide cross-department training with Performance Improvement Team.
