Job summary

KVC Health Systems, Inc. seeks an Education and Transition Specialist to provide resources to Case Managers for youth 16 and older in foster care. Hybrid work with 2 days in office; light travel required. Focus on coordinating education placement, benefits, and post-secondary planning for youth aging out of care.

Responsibilities include meetings, coordinating with schools and DCF, and compiling required reports while supporting youth with scholarships and IEPs.

Qualifications

  • Requires driver’s license and auto insurance.
  • Must be 21 years old or older (KVC Kansas policy).
  • Intermediate computer skills: typing, Word, Excel, Outlook.
  • Previous office experience and experience with high risk children or foster care preferred.

  • Schedule and host transition meetings for youth aging out of care.
  • Attend case planning conferences for youth 16+ to explain DCF benefits.
  • Complete EEISPF and send to enrolling schools.
  • Assemble transition packets outlining DCF benefit packages.
  • Locate and maintain three scholarship opportunities for ETV benefits.
  • Request IEPs for children in care.
  • Recruit and support youth at the regional council level.
  • Liaise between school district and case management team.
  • Provide Job Corp contact information for applications, interviews, and tours.
  • Assist youth with post‑secondary education applications and financial aid and tour planning.
  • Assist youth in completing extended medical card applications.
  • Participate in Best Interest Determination meetings to advocate for staying in same district.
  • Consult with DCF Coordinators to access benefit packages when leaving care.
  • Train team on Every Student Succeeds Act requirements.
  • Complete and submit required reports and monthly statistics; in-service training hours.

Join our team as an Education and Transition Specialist!

Location: This position will serve Hiawatha, KS. Full-Time, hybrid work schedule. 2 days in office required. Light travel required.

The Education and Transition Specialist provides transitional independent living resources to Case Managers for youth that are 16 and over in the foster care contract.

Position requirements:

  • A driver's license and auto insurance.
  • All KVC Kansas employees must be 21 years old or older.
  • Requires Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as Outlook email.
  • Previous office experience and experience with high risk children or foster care children is preferred.
  • Schedule and host transition meetings for youth aging out of care.
  • Attend case planning conferences for those youth that are 16 years of age and older to explain DCF benefits.
  • Complete the Education Enrollment Information for School Placement Form (EEISPF) and send to enrolling schools
  • Assemble transition packets to disseminate to Case Managers and youth that outline benefit package available through DCF.
  • Locate and maintain three scholarship opportunities for youth's qualification for ETV benefits.Request Individualized Education Plans (IEP) for children in care.
  • Recruit and support youth at the regional council level.
  • Act as a liaison between the school district and the case management team to assist in obtaining information.
  • Provide Job Corp contact information for applications, interviews, and tours.
  • Provide information for Case Managers to assist youth in applying for post-secondary education including completing financial aid packets, ACT/SAT fee waivers, tuition waiver and post-secondary education applications, scheduling tours and examining housing options.
  • At transition meetings, assist youth in completing extended medical card application.
  • Participates in the Best Interest Determination meeting with the case management team to advocate for children to remain in the same school district whenever possible.
  • Consult with DCF Coordinators for youth to access benefit package when leaving care.
  • Train team members on the requirements of the Every Student Succeeds Act.
  • Complete and submit required reports including monthly statistics.Complete required number of in-service training hours annually.
  • Support continuing education by collaborating with the case management team to obtain homework/study materials and/or access alternative options when a child is unable to attend school due to placement disruption, suspension or expulsion.
