Job summary

The role requires a Master’s Degree in Social Work from an accredited program and hospital experience, with strong skills in counseling, crisis intervention, and coordination of post-acute services.

Qualifications

• Master’s Degree in Social Work from an accredited program is required.
• 2 years hospital-based social work experience.
• Knowledge of medical terminology and disease processes; insurance regulations and requirements.

Responsibilities

• Interacts with co-workers, visitors, and other staff per Jefferson values.
• Conducts psychosocial assessment and reassessment of patients and families.
• Develops and implements intervention plans addressing identified needs.
• Collaborates with the multidisciplinary team to ensure discharge planning.
• Provides counseling to patients/families to support coping with diagnoses and end-of-life care.
