Job summary

Saint Francis Hospital in Connecticut is seeking an experienced RN Case Manager to coordinate patient care, assess needs, and develop care plans. You will collaborate with physicians and care teams to monitor progress and advocate for patients through transitions in care.

Qualifications

• Graduation from an accredited nursing school.
• Valid RN license in Connecticut.
• Bachelor's degree in Nursing preferred.
• Five years of diversified, progressive experience in acute care and/or other settings within the continuum of care.
• BLS certification required.
