Job summary

Qualifications

• Bachelor’s degree in Nursing required, or completion of Bachelor's within 18 months.
• Currently holds an unencumbered RN license in Minnesota.
• Bachelor’s degree in Social Work from an accredited curriculum required.
• Licensed Social Worker (LSW) or other allowable credential in Minnesota.

Responsibilities

• Coordinate discharge planning and connect patients with community resources.
• Facilitate communication among disciplines and agencies.
• Advocate for patients to resolve issues within the health-care system.
• Provide crisis intervention and assist families in understanding situations.
