Qualifications

• Minimum two years of RN experience in patient care delivery or completion of Masters degree in Case Management Program.
• Registered Professional Nurse License (Georgia) required.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Reviews all new inpatient admissions within 24 hours.
• Performs psychosocial assessments on all patients that meet high risk indicators.
• Acts as a liaison between inpatient facility and referral facilities/agencies.
