Job Summary

Case Manager for the Emergency Department at Bemidji Medical Center. Full‑time, Monday‑through‑Friday day‑shifts, fully benefit‑eligible. Provides integration of health care from pre‑admission to post‑discharge, coordinating and sequencing care, enhancing patient flow, promoting early intervention, and evaluating outcomes across clinical, financial, and utilization data. Works in both structured and unstructured settings and supports growth and development needs of all clients while acting as an advocate to resolve issues within the health‑care system.

Responsibilities
  • Coordinate discharge planning and connect patients with appropriate community resources.
  • Facilitate communication links among disciplines and agencies, including managed‑care case managers.
  • Advocate for patients to identify and resolve issues within the health‑care delivery system.
  • Communicate patient progress and care plans with the interdisciplinary team.
  • Ensure patients leave the Emergency Department with a clear, safe plan for next steps.
  • Integrate care coordination for clients from pre‑admission through post‑discharge.
  • Evaluate outcomes based on integration among established clinical, financial, and utilization data.
  • Function in both structured and unstructured health‑care settings.
  • Assist in growth and development needs of all client populations.
  • Provide crisis intervention, assist families in understanding medical situations, and coordinate programs among patients, families, caregivers, and health‑care teams.
  • Respond to suspected abuse, neglect, or violence in accordance with the National Association of Social Work Code of Ethics and state laws.
  • Develop appropriate plans of care for patients and families and obtain resources from social, health, and human services agencies.
  • Provide referrals, current information, and/or education regarding available programs and services.
  • Participate in care conferences and transitional rounding, serving as an advocate.
  • Demonstrate critical thinking, problem‑solving, and decision‑making skills.
  • Maintain written and verbal communication skills and establish rapport with patients, families, communities, and health‑care teams.
  • Display independent judgment and actively participate with the healthcare teams.
Qualifications
Care Manager
  • Bachelor’s degree in Nursing required, or completion of a Bachelor’s in Nursing within 18 months of hire.
  • Currently holds an unencumbered RN license in Minnesota.
Social Worker
  • Bachelor’s degree in Social Work from an accredited curriculum required.
  • Basic Life Support (BLS) certification required within six weeks of employment.
  • Licensed Social Worker (LSW) or other allowed credential in Minnesota based on the Minnesota statute.
