Qualifications

• Current RN licensure in the applicable state or jurisdiction.
• Minimum 3 years of healthcare experience, with care coordination exposure.

Responsibilities

• Coordinate discharge planning for assigned ED patients.
• Lead interdisciplinary rounds and progression huddles to ensure timely discharge.
• Collaborate with social work, CMA, and care teams to resolve barriers to discharge.
• Monitor patient progression toward discharge milestones and documentation in EHR.
