Job summary

LCMC Health in New Orleans seeks a Care Manager who serves as the hub of patient case management, coordinating care plans, transitions and communication across the care team. You will develop individualized plans and manage ongoing care activities.

You will perform outbound calls, identify gaps in treatment, coordinate referrals, and document workflows in EPIC to support quality and cost management while educating patients for self-management.

Qualifications

• Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.
• 2 years of professional nursing - Care Management experience or ED experience.

Responsibilities

• Develop individualized care plans, including prioritized goals, that consider the patient’s and caregivers’ goals, preferences and involvement.
• Make outbound calls to assess patient’s current health status.
• Identify gaps or barriers in treatment plans.
• Coordinate care for members and makes referrals to outside sources.
• Document all workflows in EPIC system for purposes of tracking and quality assessment.
• Provide patient education to assist with self-management of wellness and disease.
