Job summary

Qualifications

• Current nursing license to practice in Louisiana.
• 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans and coordinate care.
• Make outbound calls to assess patient health status.
• Coordinate care and referrals to outside sources.
• Triage symptom-based calls per approved protocols.
• Enroll patients in home monitoring programs and manage alerts.
• Document workflows in EPIC for tracking and quality.
• Educate enrolled members on chronic and acute disease processes.
• Collaborate with care team to discuss clinical questions and plans.
