Job summary

LCMC Health in New Orleans, LA is seeking a Care Manager to coordinate patient care across the continuum. You will develop individualized care plans, make outbound calls to assess health status, and coordinate hospital-to-home transitions while ensuring appropriate services and resources are available.

You will work with care team members to educate patients on self-management and disease processes, maintain documentation, and support quality and cost-management objectives.

Qualifications

• Current nursing license to practice in Louisiana as defined by LSBN.
• At least 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans with goals aligned to patient and caregiver preferences.
• Make outbound calls to assess patient health status and coordinate care transitions.
• Triage and manage symptom-based calls per approved protocols.
• Collaborate with care team and physicians to ensure appropriate services and resources.
• Educate patients on self-management and chronic disease processes.
