Qualifications

• BSN required.
• 3+ years of care coordination/case management in SMI/SUD or child welfare.
• Trauma-informed care, mental health, maternal/perinatal health, and early childhood development knowledge.
• Experience in health management or enhanced care programs desirable.
• Maternal/newborn services experience preferred.
• California RN license active.
