Job summary

Child Care Resource Center, Inc. seeks an ECM Supervisor to lead field-based health coordination across Los Angeles and San Bernardino counties. The role requires a California RN license, a BSN, and at least 3 years in care coordination, with bilingual English/Spanish proficiency. Travel up to 25% and community engagement are expected.

You will supervise the ECM team, ensure compliance, train staff, and provide health consultations to families in homes, clinics, and Head Start centers.

Qualifications

• Current California RN license.
• Bachelor of Science in Nursing.
• Minimum three years in care coordination or case management.
• Bilingual English/Spanish required.
• Knowledge of trauma-informed care and maternal/early childhood health.
• Ability to travel up to 25% within LA/San Bernardino counties.

Responsibilities

• Oversee ECM program operations and supervise staff.
• Ensure documentation, compliance and reporting requirements are met.
• Coordinate program operations and collaboration with partners.
• Provide telephonic/virtual nursing consultation to families.
• Train staff on child health conditions and procedures.
• Represent CCRC at community events and Head Start settings.
