RHA Health Services, LLC is seeking a CAP Clinical Supervisor for the East Region. The role focuses on supervising and coordinating CAP case management for medically complex clients living at home while ensuring adherence to Medicaid and CAP guidelines.

• Bachelor’s degree in social work from an accredited school of social work.
• Minimum of five years of directly related experience, preferably CAP case management.
• Bachelor’s degree in human services or equivalent field and five years of community experience.

Responsabilidades

• Complete initial and annual assessments.
• Facilitate CAP case management services with high quality and compliance.
• Oversee services to ensure alignment with federal/state regulations and CAP guidelines.
• Provide training and development to staff on clinical documentation and practice.
• Lead team meetings and contribute to Quality Improvement programs.
