Job summary

Qualifications

• High school diploma or equivalent with three years' experience in office, administrative, or customer service.
• College coursework or vocational training may substitute for the required experience.

Responsibilities

• Intake and assess eligibility, psychosocial history, and demographics for the Early Intervention Program.
• Provide family support planning and coordinate with a multi-disciplinary team.
• Arrange appointments, transportation, and coordinate care for Part C eligible children and families.
• Document contacts in EIP records and Family Support Plans; bill Medicaid as required.
