Job summary

Mount Rogers Community Services is seeking an Early Intervention Professional to coordinate services, determine eligibility for Part C, and support families through IFSP processes. You will work within a multidisciplinary team to ensure services are delivered in the child’s natural environment and aligned with coaching principles.

The role requires licensure in the relevant discipline, proficiency in documentation, and effective communication with families and providers.

Qualifications

• Must meet discipline-specific licensure/certification requirements.
• Strong understanding of IFSP development and service coordination processes.
• Ability to work with interagency teams and families in coaching model delivery.

Responsibilities

• Conduct eligibility determinations for Part C Early Intervention Services.
• Gather and review documentation from referrals and medical providers.
• Maintain IFSP documentation, service coordination, and progress notes.
• Facilitate transitions when Early Intervention is no longer needed.
• Attend meetings and provide reports on client progress and service coordination.
