Job summary

Central Counties Services is seeking a qualified social services professional to serve as a liaison between families and service providers for ECI programs. You will conduct intake, schedule evaluations, and coordinate IFSP development with a transdisciplinary team while ensuring timely service delivery.

Responsibilities include developing child outcomes, documenting progress, and supporting families in navigating the ECI system. Regular driving in the catchment area is required.

Qualifications
