Qualifications

• Master’s Degree in Social Work; current licensure as LCSW or LICSW
• Two years pediatric experience preferred
• Effective verbal and written communication skills
• Working knowledge of Word, Excel, Adobe, and Office365 (Outlook/OneDrive/Teams)

Responsibilities

• Demonstrate knowledge of infant-toddler development
• Utilize a parent coaching model with families
• Participate in screenings, assessments, child groups, parent groups, center visits, and home visits as part of a trans-disciplinary team
• Provide home-based services to families across the catchment area
• Collaborate with community providers to ensure continuity of services for families including child care, medical, social services, and autism/vision/hearing supports
• Provide service coordination to assigned caseload and develop/monitor/review IFSP
