Job summary

Qualifications

• Two years of experience in a health or human services field.
• 24 semester hours of education in behavioral sciences, social sciences, or a related field.
• A combination of education and experience equating to two years in a related field.

Responsibilities

• Meet with families to review needs and available supports.
• Help families access services and community resources.
• Coordinate referrals and communication with service providers.
• Assist with Individualized Family Service Plans (IFSPs).
• Schedule and lead IFSP team meetings.
• Support families in advocating for their child's needs.
• Maintain accurate records as per DHHS guidelines.
