POSITION TITLE: Early Intervention Service Coordinator
POSITION LOCATION: Child and Family Connections #16 – Champaign County, Illinois
POSITION TYPE: Full-Time (40 hours/week)
POSITION STATUS: Work from home with State of Illinois residency requirements: candidates must live in Champaign County, Illinois to be considered for this position
Qualifications
- Candidates are required by the State of Illinois to have a Bachelor's degree or higher from an accredited college or university in Education, Social Work, Human Services, Behavioral Science, or a health-related field

- Eligible for Early Intervention Service Coordination Credential
- Proven successful experience working with children and families preferred
Child and Family Connections of Central Illinois is the regional intake agency for children birth to three and their families to enter the Illinois Early Intervention System. Early Intervention (EI) credentialed Service Coordinators work with families to develop an Individualized Family Service Plan and select service providers to meet the individualized needs of their child. Child and Family Connections can also assist with Social Emotional Consultative Services and free developmental screenings through the Local Interagency Councils.
Position Objective
The Early Intervention Service Coordinator provides in-home or center-based consultation services and are responsible for utilizing a variety of formalized assessment tools for the child and/or other family members. The Service Coordinator coordinates with other disciplines and agencies to ensure a comprehensive assessment; reviews family resources, relationships and attitudes, and makes independent clinical judgements regarding appropriate services to be provided to client and family.
Job Functions
- Conducts intake interview with family and complete required documentation. Offers provider options for families in order that they might select a provider to evaluate and serve their child and family.
- Participates in the Individualized Family Service Plan meetings to determine child eligibility and functional outcomes.
- Manages and monitors financial and medical services, including insurance coordination, for enrolled children as determined by the IFSP.
- Informs families of rights and the availability of community resources such as housing, transportation and meaningful activities.
- Informs families of information regarding State programs such as KidCare.
- Issues accurate and timely authorizations to Early Intervention providers for all services.
- Maintains caseload list with providing regular client communication and completing all Early Intervention reporting requirements within the Cornerstone system.
- Attends Case Review meetings with the Medical Consultant and the Social Emotional Consultant to increase education of developmental delays, disabilities, and needs of clients.
- Initiates transition from Early Intervention to school services as required by DHS, Bureau of Early Intervention.
Marginal Responsibilities
- Distributes document(s) to team and other providers identified in the plan.
- Attends LIC, AOK and other meetings throughout the region as appropriate.
- Fixes authorizations and assist in other cornerstone data corrections as needed.
- Conducts developmental screenings on an as-needed basis.
- Performs other duties as assigned.

- Must have visual ability sufficient to read and complete written correspondence, reports, etc., and computer screen information.
- Frequent use of hands to grasp, handle, or feel.
- Regular standing, walking, sitting, and reaching.
- Occasionally lift and/or carry up to twenty-five (25) pounds.
- Ability to drive personal vehicle to meetings and service sites.
Work Environment
- Primarily operates in office and home environments with little exposure to excessive noise, dust, temperature.
- Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
