Job summary

Mentor South Bay is seeking an Early Intervention Physical Therapist to coordinate treatment and implement services for assigned clients as part of a multidisciplinary Early Childhood Services team.

You will provide outreach services, develop individualized plans with the team, and maintain complete documentation for IFSPs and client progress, while ensuring billing and compliance with regulations.

Qualifications

• Licensed Physical Therapist in Massachusetts.
• Experience coordinating treatment and outreach visits.
• Ability to document client progress and IFSPs accurately.
