Job summary

Mentor South Bay in Leominster, MA is seeking an Early Intervention Occupational Therapist to coordinate treatment and deliver services within a multidisciplinary Early Childhood Services team.

You will provide outreach to children and families, develop individualized plans with your team, document IFSPs and progress, manage billing paperwork, and ensure compliance with DPH regulations while maintaining client confidentiality.

Qualifications

• Current licensure as an Occupational Therapist by the MA Board of Registration of Allied Health Professionals.
• Experience collaborating with multidisciplinary Early Childhood teams.

Responsibilities

• Provide outreach services to children and families in the community.
• Consult with team members to develop individualized plans and multidisciplinary services.
• Maintain client records including IFSPs with full assessment of functioning and goals.
• Document client progress and required contact with collaterals and families.
• Complete billing documentation according to company schedules and guidelines.
• Adhere to professional standards and maintain confidentiality.
• Ensure visits align with part-time or full-time status and regulatory requirements.
