Job summary

Mass General Brigham is seeking a Patient Navigator for the Massachusetts Eye and Ear Infirmary to support infants and toddlers birth–3 with deaf or hard of hearing needs. You will coordinate diagnostic and Early Intervention services, guide families through care pathways, and liaise with EI agencies and Deaf/Hard of Hearing providers.

Responsibilities include counseling families, maintaining documentation, tracking appointments, and ensuring adherence to EHDI timelines.

Qualifications

Bachelor's degree in Social Work, Healthcare Management, or related field.
Experience in social work, case management or related field preferred.
Ability to coordinate care across multidisciplinary teams.

Responsibilities

Advocate for patients and families, ensuring needs are considered in care planning.
Serve as liaison between patients, providers, and stakeholders.
Coordinate care with audiology, SLP, EI providers to organize services and follow-up.
Counsel caregivers on hearing loss and intervention pathways.
Guide families through the Pediatric Hearing Healthcare Roadmap.
Monitor timelines for EHDI diagnosis and intervention; ensure timely referrals.
Document interactions, care plans, and resource referrals in records.
Participate in case reviews and contribute to care planning.
