The role emphasizes family-centered care, collaboration with an interdisciplinary team, and adherence to EHDI timelines. Onsite work is required in a pediatric hearing healthcare setting.

Qualificações

• Bachelor's Degree in Social Work or Healthcare Management required.
• Experience in social work, case management, or related field preferred.
• Licenses and credentials: Social work, case management or related field.

Responsabilidades

• Advocate for patients and families, ensuring needs are considered in care planning.
• Serve as liaison between patients, families, healthcare providers, and stakeholders.
• Coordinate care with audiology, speech-language pathology, EI providers, and other teams.
• Assist families in understanding care plans and intervention timelines for birth–3 with hearing loss.
• Identify and connect families with healthcare and community resources (financial assistance, transportation).
• Document patient interactions, care plans, and referrals with accuracy.
