Cumberland Mountain CSB is seeking a Part-time Early Intervention (EI) Case Manager to provide service coordination for infants and toddlers in Buchanan, Russell and Tazewell Counties. Responsibilities include screening, coordinating IFSP supports, and maintaining contact with families by phone or in person.

Applicants must meet education requirements, complete a state competency test, have a valid driver's license and pass background screenings.

Qualifications

  • Bachelor's degree in Allied Health, counseling, early childhood, social work or related field.
  • Associate's degree in a related field such as OT/PT aide or nursing.
  • High school diploma with 3 years of full-time experience coordinating direct services.

Description

Cumberland Mountain CSB is seeking a professional for an exciting career as a Part-time Early Intervention (EI) Case Manager. The EI Case Manager will provide service coordination to developmentally delayed infants and toddlers who reside in Buchanan, Russell and Tazewell Counties. Duties include: Screening for services, oordinate and monitor the delivery of IFSP (Individualized Family Service Plan) Supports and Services, make direct monthly contacts with families by telephone or face to face, request needed information from physicians and other service providers, document all contacts made and all activities completed with or on behalf of children and families, hold IFSP reviews as needed, and monitor Medicaid and insurance coverage issues. Rate of pay is based on education and experience. Will work up to 29hours per week, depending upon need.

Hiring Range

Hiring range: $17.50- $26.25 hourly. (Offers for candidates with more than 20years of relevant experience may exceed the listed hiring range. Cumberland will only engage in compensation discussions and negotiations after an initial offer of employment is extended.)

Position Requirements

Applicants must hold one of the following:

  • A minimum of a Bachelor's degree in one of the following fields:
    • Allied Health, including rehabilitation counseling, recreation therapy, occupational therapy, physical therapy, or speech or language pathology
    • Child and Family Studies
    • Counseling
    • Early Childhood
    • Early Childhood growth and development
    • Early Childhood special education
    • Human Development
    • Human Services
    • Music Therapy
    • Nursing
    • Psychology
    • Social Work
    • Special Education - Hearing Impairments
    • Special Education - Visual Impairtments
    • Other related field or interdisciplinary studies approved by the State Lead Agency; or
  • An Associate's Degree in a related field such as occupational therapy assistant, physical therapy assistant or nursing; or
  • A high school diploma or GED, or an undergraduate degree in an unrelated field, plus three years of full-time experience coordinating direct services to children and families and implementing individual service plans. Direct services address issues related to developmental and physical disabilities, behavioral health or educational needs, or medical conditions. Experience may inlcude supervised internships, practicums, or other field placements.

The applicant must complete a competency test (state provided) that details early intervention services and service coordination.

All candidates must also possess a valid driver’s license and good driving record. Candidate for hire must also pass all CMCSB background screenings.

About the Organization

CMCSB was formed in 1972 as a Chapter 10 Community Services Board under Virginia law. CMCSB provides mental health, intellectual disabilities and substance abuse treatment services to the residents of Buchanan, Russell, and Tazewell Counties. At the present time, CMCSB operates over twenty locations, to include outpatient offices, mental health recovery programs, group homes and day support programs for intellectually disabled individuals, and a residential alcohol and drug treatment program. In addition, CMCSB has several services that are provided directly in an individual's home or in a supported environment. CMCSB also provides a 24-hour crisis team to assist community agencies and individuals in crisis situations. CMCSB employs approximately 500 employees, which include full-time and part-time employees, as well as individuals receiving services who are employed to work in various day programs.
