Cumberland Mountain CSB is seeking a professional for an exciting career as a Part-time Early Intervention (EI) Case Manager. The EI Case Manager will provide service coordination to developmentally delayed infants and toddlers who reside in Buchanan, Russell and Tazewell Counties. Duties include: Screening for services, oordinate and monitor the delivery of IFSP (Individualized Family Service Plan) Supports and Services, make direct monthly contacts with families by telephone or face to face, request needed information from physicians and other service providers, document all contacts made and all activities completed with or on behalf of children and families, hold IFSP reviews as needed, and monitor Medicaid and insurance coverage issues. Rate of pay is based on education and experience. Will work up to 29hours per week, depending upon need.
Hiring range: $17.50- $26.25 hourly. (Offers for candidates with more than 20years of relevant experience may exceed the listed hiring range. Cumberland will only engage in compensation discussions and negotiations after an initial offer of employment is extended.)

CMCSB was formed in 1972 as a Chapter 10 Community Services Board under Virginia law. CMCSB provides mental health, intellectual disabilities and substance abuse treatment services to the residents of Buchanan, Russell, and Tazewell Counties. At the present time, CMCSB operates over twenty locations, to include outpatient offices, mental health recovery programs, group homes and day support programs for intellectually disabled individuals, and a residential alcohol and drug treatment program. In addition, CMCSB has several services that are provided directly in an individual's home or in a supported environment. CMCSB also provides a 24-hour crisis team to assist community agencies and individuals in crisis situations. CMCSB employs approximately 500 employees, which include full-time and part-time employees, as well as individuals receiving services who are employed to work in various day programs.
