Job summary

Cumberland Mountain CSB is seeking a Part-time Early Intervention (EI) Case Manager to provide service coordination for infants and toddlers in Buchanan, Russell and Tazewell Counties. Responsibilities include screening, coordinating IFSP supports, and maintaining contact with families by phone or in person.

Applicants must meet education requirements, complete a state competency test, have a valid driver's license and pass background screenings.

Qualifications

• Bachelor's degree in Allied Health, counseling, early childhood, social work or related field.
• Associate's degree in a related field such as OT/PT aide or nursing.
• High school diploma with 3 years of full-time experience coordinating direct services.

Responsibilities

• Provide service coordination to developmentally delayed infants and toddlers in Buchanan, Russell and Tazewell Counties.
• Screen for services, coordinate and monitor IFSP supports and services.
• Make direct monthly contacts with families by telephone or face-to-face and document all activities.

Education

Bachelor's degree in Allied Health / related fields

Associate's Degree in related field (OT/PT aide, nursing)

High school diploma or GED plus 3 years coordinating direct services
