Job summary

Easterseals Hawaii in Hilo, HI seeks a Care Coordinator (EI) to support families of children aged 0-3 with developmental delays. You will coordinate services, educate families, and monitor progress as part of a multidisciplinary team within an Early Intervention program.

Responsibilities include family coaching, collaboration with the Hawaii DOH EI section, and maintaining service plans and documentation, while reporting to the EIS program manager.

Qualifications

• Bachelor’s degree in Social Work, Social Service, Education or equivalent from an accredited college or university program.
• Active LSW or LCSW license REQUIRED
• One (1) year minimum experience in professional social work is highly preferred
• Experience working with children with special needs in the field of early intervention is highly preferred

Responsibilities

• Coordinates child development monitoring and intervention services to children ages 0-3 with developmental delay or disability.
• Models strategies and provides education to parents/caregivers to promote development.
• Collaborates with the team to create and monitor child outcomes on the IFSP.
• Maintains program guidelines, timelines, and caseload reporting with funders.
• Meets productivity requirements and provides intervention strategies and family resources.
