Qualifications

• Bachelor's degree in Social Work or equivalent.
• Active LSW or LCSW license REQUIRED.
• One year minimum experience in professional social work preferred.
• Experience with children with special needs in EI is highly preferred.

Responsibilities

• Coordinate child development monitoring and intervention services for 0–3 years old with delays or disabilities.
• Model strategies and educate parents/care providers to promote development.
• Collaborate to set and monitor child outcomes on the Individualized Family Service Plan (IFSP).
• Coordinate with Hawaii DOH EI Section and meet funder guidelines and reporting requirements.
• Meet productivity targets for the caseload and provide intervention strategies and resources.
• Facilitate weekly Family Support Team meetings under the EI Program Manager.
• Schedule home visits or telepractice visits based on family preference.
• Coordinate family visits with other providers as needed.
• Assess, evaluate, and consult on child developmental progress using evidence-based tools.
• Complete billing and service plan documentation within timelines.
• Provide written and verbal communications to families and care providers.
• Explain Primary Service Provider Coaching model and support families in coaching to foster development.
• Offer other resources and referrals as needed.
• Stay current with new research and developments in special education.
• Complete additional duties as assigned by management.
