Qualifications

• RN required (PA) upon hire; NJ license also required.
• Must have pediatric nursing experience.
• Experience in care coordination and nurse navigator roles preferred.

Responsibilities

• Coordinate care with referring clinicians, patients and families to ensure access and ongoing communication.
• Manage plan of care for referral patients and serve as contact with referring physicians.
• Assess health status and psychosocial needs; provide disease education and care coordination.
• Assist scheduling, triage new patients, and allocate to providers for multi-appointment care.
• Develop outreach materials and collaborate with CHOP departments to strengthen referrals.
• Establish and maintain metrics and reports for program growth.
