Qualifications

• Active Connecticut Physical Therapist license required.
• Strong assessment, treatment planning, and clinical documentation skills.

Responsibilities

• Evaluate functional mobility, strength, balance, and range of motion, considering medical history and precautions.
• Set measurable rehabilitation goals and establish treatment plans for each resident.
• Provide therapeutic exercise, gait training, and functional activities to support daily movement.
• Assess mobility equipment needs and instruct residents and caregivers in safe use.
• Review treatment outcomes, modify care plans, and oversee PTA participation in resident services.
• Complete clinical documentation and contribute mobility recommendations to interdisciplinary care and discharge planning.
