Job summary

Stern Therapy Consultants is seeking a PRN Physical Therapist to provide coverage at Autumn Lake at Windsor in Windsor, CT. You’ll help residents in a skilled nursing setting with evaluations, individualized treatment, and coordination with the care team.

Active Connecticut Physical Therapist license required; strong clinical judgment and documentation skills; SNF experience is a plus. PRN opportunities based on facility needs.

Qualifications

Responsibilities

• Evaluate residents after hospital stays or changes in mobility.
• Develop treatment plans focused on strength, balance, walking, and transfers.
• Provide therapy and reassess progress as residents' needs change.
• Communicate mobility recommendations to nursing and other clinicians.
• Complete evaluations, progress notes, and treatment documentation.
