Job summary

PruittHealth Hospice is seeking a Nurse Practitioner for PRN coverage in Georgia, providing hospice and palliative care in patients' homes or care facilities. You’ll conduct adult/geriatric assessments, manage plans of care, and collaborate with an interdisciplinary team.

Qualifications

Current, active RN licensure in state of practice.
Current, active NP licensure in state of practice.
Board-certified by AANP or ANCC in adult, family, geriatric, or adult-gerontology primary care.

Responsibilities

Proactive, collaborative team member in a hospice care environment.
Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
Commitment to caring for patients and partners.
Complete all documentation and records in a timely and accurate manner.
Follow established policies and procedures as related to the scope of practice.
Maintain continuity of patient care in a hospice setting.
