Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Trotwood, OH. The role supports a Physical Therapist and focuses on resident-driven care and functional outcomes.

Qualifications

• Active license as Physical Therapy Assistant in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying a target list, including short-term goals, long-term goals and treatment plan.
• Conduct both individual and group treatment as assigned by the Physical Therapist and outlined in treatment plan including strengthening, mobility, balance, gait training, and ROM activities.
• Assist in selecting physical therapy techniques/media and sequence of activities; adapt techniques/media per resident performance in consultation with the PT.
• Monitor residents’ response to intervention and consult with the PT if modification is indicated.
• Assist in establishing and modifying individual resident schedules with the PT and team members.
• Adhere to department and resident schedule and modify as appropriate for treatment regimen.
• Train resident, family and appropriate staff in equipment use or application.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Report to work on time and coordinate schedule for productivity and efficiency during assigned shift.
• Assist with QA programs as required.
• Complete CMS-compliant and medically necessary documentation in NetHealth EMR.
