Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Trotwood, OH. This PRN role focuses on delivering high-quality PT services and supporting residents in a team-based setting.

You will perform evaluations, develop treatment plans, and document per CMS requirements using NetHealth EMR, while maintaining ongoing professional development and licensure.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsabilités

• Provide physical therapy services to residents per established standards.
• Adhere to documentation and billing procedures for PT services.
• Evaluate residents to inform treatment planning and intervention.
• Develop short and long-term treatment goals and plans.
• Communicate evaluation findings and recommendations to residents and team.
• Complete CMS-compliant documentation in NetHealth EMR.
