Job summary

Evolve Therapy Services seeks a Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Trotwood, OH. This PRN role focuses on delivering patient-centered physical therapy under supervision and ensuring CMS-compliant documentation.

Responsibilities include screenings, treatment plan support, and collaboration with the Physical Therapist to optimize resident mobility, strength, and safety. Licensure in good standing and ongoing education are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluation and development of treatment plans with the Physical Therapist.
• Participate in patient and family education.
• Document in NetHealth EMR per CMS requirements.
• Coordinate resident schedules with the Physical Therapist and team.
