Job summary

Qualifications

• Certification: Basic Life Support (BLS) within 30 days – American Heart Association (AHA).
• Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR).
• Education: Bachelor's Degree in Physical Therapy (Required).

Responsibilities

• Transfers: demonstrates ability with assistance from bed, chair or floor; uses safe body mechanics.
• Designs and adjusts plan of care to meet goals, outcomes, and discharge planning.
• Educates patients, families, caregivers, students and staff using effective teaching methods.
• Performs examinations, determines PT diagnoses and outcomes; documents per CTSCP67.
• Communicates progress to referring providers and documents in therapy EMR.
