Job summary

Clover Health Services in Oakland, California is seeking a Physical Therapist to evaluate patients, develop individualized treatment plans, and guide rehabilitation to restore mobility and function.

You will administer exercises, manual therapy, and neuromuscular re-education; monitor progress and discharge with home programs. The role requires collaboration with physicians and care teams, supervision of PTAs, and thorough documentation during a 13-week, day-shift assignment starting mid-August.

Responsibilities

• Evaluate patients' physical condition, movement limitations, pain levels, and functional deficits.
• Develop and implement individualized treatment plans to restore mobility, strength, and functional ability.
• Administer therapeutic exercises, manual therapy, modalities, and neuromuscular re-education.
• Treat patients recovering from injury, surgery, or managing chronic musculoskeletal conditions.
• Monitor patient progress, adjust interventions, and ensure measurable outcomes.
• Educate patients on injury prevention, posture, ergonomics, and home exercise programs.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately.
• Supervise and delegate appropriate tasks to Physical Therapist Assistants (PTAs) and therapy aides.
• Communicate effectively with physicians, caregivers, and interdisciplinary healthcare teams.
• Discharge patients with individualized maintenance and self-management plans to support long-term recovery.
