Job summary

Clover Health Services in Castro Valley, CA is seeking a Physical Therapist to evaluate patients, develop treatment plans, and administer therapies to restore mobility and function.

You will document progress, supervise PTAs, communicate with physicians, and provide discharge planning for long‑term recovery. Shifts 7:00 AM to 3:00 PM, start date 2026-09-28, duration 13 weeks, weekly pay.

Responsibilities

• Evaluate patients physical condition, movement limitations, pain levels, and functional deficits.
• Develop and implement individualized treatment plans to restore mobility, strength, and functional ability.
• Administer therapeutic exercises, manual therapy, modalities, and neuromuscular re-education.
• Treat patients recovering from injury, surgery, or managing chronic musculoskeletal conditions.
• Monitor patient progress, adjust interventions, and ensure measurable outcomes.
• Educate patients on injury prevention, posture, ergonomics, and home exercise programs.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately.
• Supervise and delegate appropriate tasks to Physical Therapist Assistants (PTAs) and therapy aides.
• Communicate effectively with physicians, caregivers, and interdisciplinary healthcare teams.
• Discharge patients with individualized maintenance and self-management plans to support long-term recovery.
