Job summary

Clover Health Services in Rocklin, California, is seeking a Physical Therapist to evaluate patients, develop individualized treatment plans, and help restore mobility, strength, and functional ability.

The role involves administering therapeutic exercises and manual therapies, monitoring progress, educating patients, and coordinating with physicians to ensure optimal recovery. This is a 13-week contract with daytime shifts.

Responsibilities

• Evaluate patients physical condition, movement limitations, pain levels, and functional deficits.
• Develop and implement individualized treatment plans to restore mobility, strength, and functional ability.
• Administer therapeutic exercises, manual therapy, modalities, and neuromuscular re-education.
• Treat patients recovering from injury, surgery, or managing chronic musculoskeletal conditions.
• Monitor patient progress, adjust interventions, and ensure measurable outcomes.
• Educate patients on injury prevention, posture, ergonomics, and home exercise programs.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately.
• Supervise and delegate appropriate tasks to Physical Therapist Assistants (PTAs) and therapy aides.
• Communicate effectively with physicians, caregivers, and interdisciplinary healthcare teams.
• Discharge patients with individualized maintenance and self-management plans to support long-term recovery.
