Job summary

PACS Group in Santa Cruz is seeking a dedicated Physical Therapist to provide patient-centered rehab services in a collaborative setting. You will evaluate, plan, and deliver skilled PT to restore strength, mobility, balance, and functional independence for residents.

We offer full-time, part-time, and PRN options with starting pay of $50–$58/hour DOE, plus benefits including medical, dental, vision, PTO, retirement options, CE support, and wellness programs.

Qualifications

• Current Physical Therapist license in California.
• Graduate of an accredited Physical Therapy program.
• Strong communication, documentation, and time-management skills.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans for residents with physical and functional impairments.
• Provide therapeutic interventions including gait training, strengthening, balance activities, pain management, and mobility training.
• Document evaluations, daily notes, progress reports, and discharge summaries in accordance with regulatory and facility standards.
• Collaborate closely with nursing staff, therapy team, physicians, and families to support resident goals.
• Educate residents and caregivers on exercise programs, safety techniques, and mobility strategies.
• Participate in interdisciplinary care conferences and contribute to overall care planning.
• Maintain compliance with state, federal, and professional practice regulations.
