Job summary

Concentra is seeking a Physical Therapist in Fresno, CA to evaluate patients, supervise clinical activities, and help restore motor function and daily living skills within an occupational medicine framework.

You will collaborate with a multidisciplinary team, document patient care, and support rehabilitation goals while ensuring compliance with healthcare regulations and quality standards.

Qualifications

• Bachelor’s Degree from an accredited Physical Therapy program.
• Masters’ Degree from an accredited Physical Therapy program, preferred.
• Knowledge of HIPAA and patient privacy requirements.
• Experience documenting patient care and coordinating across disciplines.

Responsibilities

• Ensure delivery of high quality patient care services.
• Perform direct patient care, including evaluation of muscle, skeletal, neurological and other physical problems.
• Support and encourage multidisciplinary coordination of treatment.
• Ensure proper documentation of patient care.
• Maintain professional standards through compliance with quality assurance policies and applicable regulations.
• Communicate with staff and patients to ensure coordinated care and optimal outcomes.
• Educate patients in therapies and home exercise programs.
• Collaborate with physicians to ensure comprehensive delivery of services.
